WE CAN ASSIST IN THE MANAGEMENT OF PATIENTS BY:
Performing a comprehensive assessment by the appropriate discipline that encompasses:
- Physical Status
- Home environment and family support systems
- Educational needs relative to disease process, symptoms, complications, medications, and treatment.
- Individualize a plan of care, and submit status reports to physicians on the patients condition and response to treatment

RESPIRATORY CARE
- Rehabilitation
- Evaluation and teaching
- Oxygen saturation monitoring
- Breath conservation technique
CARDIAC CARE
- Medication administration
- Cardiac teaching
- Rehabilitation
WOUND CARE
- Wound assessment
- Sterile dressing changes
- Whirlpool treatment
DIABETIC CARE
- Glucose monitoring and teaching
- Teaching to include: process, self assessment, foot care and diet complications
DECUBITIS CARE
- Treatment per physicians orders and protocol
- Intensive instruction to family on treatment and prevention
OTHER AVAILABLE SERVICES:
- Medication administration; IV therapies to include: Antibiotic, TPN, hydration, and pain management
- IM and SC injections
- Enterostomal therapy
- Insertion and or/maintenance of: catheter lines, foleys, and nasogastric tubes
- Management of high risk patients, i.e. oncology, multi-system failure
- Pulmonary rehabilitation
- Comprehensive assessment of deficits by a speech therapist, including swallowing ability and individualized programs that encompass all aspects of communication
- Individualized rehabilitation program by OT's or PT's including muscle reeducation, fine motor skills, transfers, balance, gait, ROM and other coordination activities
- Assistance with ADL's
- Custom splinting

ADDITIONAL INFORMATION
- Home care is initiated upon physician's orders
- Care is given during regularly scheduled visits in the patient's place of residence
- The patient must be homebound, i.e. unable or require assistance to leave the home in order to be eligible for home care services
PAYMENT
- Medicare, Medi-Cal, and other insurance carriers provide home health care coverage
- Our staff will obtain authorization for visits, and bill the insurance company for services provided
- In most cases the patients do have to make any payments for services provided
